Elderly Care Application Process

  • Signed application form submitted by the family

  • Completion of assessment information

  • Submission of medical information that will assist in the individualized program plan

  • Provide living will with DNR order if applicable

  • Payment of registration fee

Eligibility Criteria for Elderly Care

Appropriate clients are those who cannot be safely left alone at home and live with someone who either works outside the home or needs respite. Client admission will be viewed as 30-day trial for both parties after this time the decision will be made about client’s continued attendance

   Eligibility Criteria is as follows:

  • 55 years or older

  • Able to participate in Center activities

  • Able to communicate needs and wants to staff

  • Able to walk alone or use an assistive device(walker, cane, wheelchair)

  • Not prone to inappropriate behaviors or violent outbursts

  • Able to socialize in a cooperative, non-harmful manner(no disruptive, violent or abusive behavior)

  • Documentation by Successful Living Center indicating that individual’s behavior is manageable in accordance with requirements set by Successful Living Center

  • Free of substance abuse

  • Manageable incontinence (may wear diaper or pad)

  • Strength/stamina to attend a day program for at least 4 hours

  • Documented evaluation by physician indicating diagnosis of Alzheimer’s or related dementia (must be provided before admission into program)

  • Recent medical examination (dated within 6 months prior admission) certifying participant is free of contagious disease (on file prior to first day of program participation).

 

Persons will not be accepted or services may be discontinued for any of the following:

  • Requires constant one-to-one attention

  • Disruptive to overall group or causes constant anxiety to other participants

  • Severe confusion

  • Center is unable to meet the needs of the applicant, as determined by the physician, participant, or caregiver

  • Non-payment of fee

  • Contagious disease or infection

  • Not returning signed medical forms from the physician

  • Not observing policies as set forth in the Center’s manual

  • Condition progresses whereby participant requires one-to-one attention

An assessment of participant’s needs will be made at the time of admission. The assessment will be reviewed and updated as needed. Those persons with exceptional needs will be evaluated on an individual basis.

Cancellation Policy

Cancellation of attendance should be made at least 24 hours in advance whenever possible. If cancellation is due to a sudden illness, the Center should be informed at least one half hour prior to the time the client is scheduled to arrive at the Center. Messages may be left on our voice mail at any time of the day.
